We operate a WRITTEN REQUEST System for Prescriptions.
Our practice staff are unable to take prescription requests over the phone.
Here are 3 ways you can request a prescription:
1.Fill in an online form
If you do not have access to a computer/phone/tablet or printer, a repeat prescription form can be picked up from reception.

2: Written request
Drop a written request into reception or place in the post-box outside the surgery.

Post your request form to the surgery at:

How do I obtain medication that a specialist or hospital recommended?
Your pharmacy will be able to dispense a 7-day emergency supply, if you attend the
pharmacy on the day the prescription is given. Then drop the prescription to our surgery so
that we can update your records and email it on to the pharmacy.
